Professor Yu Buwei is a leading anesthesiologist with international influence and innovation in contemporary Chinese anesthesiology. He established the theory of "ideal anesthesia state" and the standard of "precise anesthesia", rapidly improving the safety level of anesthesia in China; We have established innovative concepts and practices such as "comfortable medical care", "anesthesia therapy", and "artificial intelligence anesthesia", expanded the field of anesthesia medicine and service quality, and first created anesthesia treatment disciplines and programs for refractory diseases internationally; "Anesthesiology treatment plan for critically ill patients infected with novel coronavirus" was established, which effectively improved the rescue success rate of critically ill patients infected with novel coronavirus. The establishment of the principle of "bilateral reciprocal exchange" with international academic organizations has led Chinese anesthesiologists to the world, enhancing the overall level and international status of Chinese anesthesiology.

 

 

Professor Buwei Yu is a leading anesthesiologist with international reputation and innovation in contemporary Chinese Society of Anesthesiology. He has established the clinical theory of "Ideal State of Anesthesia" and the standard practices of "Accuracy Anesthesia", rapidly improving the safety level of clinical anesthesia in China. Professor Yu has established many innovative concepts and practices such as "comfortable medical care", "anesthesia therapy", and "artificial intelligence anesthesia", expanded the field and concepts of anesthesia medicine and its service quality, and take the lead in creating the Anesthesia Treatment and Therapy Clinic for the refractory diseases internationally. Professor Yu has established the anesthesia rescue therapies for severe and critically ill SARS-CoV-2 infection patients, which effectively improved the rescue success rate of critically ill SARS-CoV-2 infection patients, Yu’s establishment of the principle of "bilateral reciprocal exchange" with international academic organizations has led Chinese anesthesiologists to the world stage, enhancing the overall levels and international status of Chinese Anesthesiology.

1、 Pioneering the concepts and standards of "Ideal State of Anesthesia" and "Accuracy Anesthesia" internationally, and building the first set of clinical anesthesia practices standards and guidelines of Chinese Society of Anesthesiology, that based on the "Ideal State of Anesthesia" theory and "Accuracy Anesthesia" practices, and greatly promoting the overall rapid improvement of clinical anesthesia safety in China.

In the early 1990s, Professor Yu took the lead in establishing a modern anesthesia department that integrated clinical anesthesia, critical care medicine, and pain medicine during his tenure at the former Changhai Hospital of the Second Military Medical University. He achieved digital medicine, electronic cases, standard prescriptions, and modern ICU wards for the first time, demonstrating and leading the modernization process of anesthesiology in China.

In 1999, Professor Yu proposed the theoretical and practical items of "Ideal State of Anesthesia" and "Accuracy Anesthesia", with the core concepts as maintaining stable internal environment and microcirculation function in effective monitoring of depth and stress levels, making clinical anesthesia a therapeutic process; The traditional model of anesthesia relying solely on vasopressors to maintain blood pressure has been changed by Yu’s new theory and concepts, and an accuracy anesthesia protocol has been created that scientifically manages effective blood volume, tissue perfusion, and microcirculation function. The novel theory and practices have been widely accepted and enhancing the safety level and development ability of Chinese anesthesiology discipline. Breaking through the traditional boundary of "anesthesia taboos", it has won the opportunity for surgical treatment of critically ill and special age patients, and effectively ensuring the safe conduct of complex and high-risk surgeries. The improvement of the overall strength of the anesthesia department effectively guaranteeing the development of the surgical department and other disciplines. The theoretical and practical items of "Ideal State of Anesthesia" and "Accuracy Anesthesia" have also been included in anesthesia textbooks and clinical guidelines. Professor Yu has invented clinical equipment as anesthesia accuracy monitoring, nociception monitoring, and peripheral microcirculation function monitoring based on the "Ideal State of Anesthesia" and "Accuracy Anesthesia" theory and practice.

In 2010, the first anesthesia depth EEG monitoring theory and technology training center of CAS was established, which has trained more than 2000 anesthesiologists nationwide, promoting the transformation of anesthesia in China from empirical medicine to accuracy science.

Professor Yu has promoted the establishment of clinical anesthesia quality control and safety standards, and conducted a large-scale domestic survey on postoperative neuromuscular blockers residual effects. It has been confirmed for the first time that the incidence of postoperative neuromuscular blockers residual in China is highly as 57.8% (Current Medical Research Opinion, 2016: 1). Yu’s suggestion as the routine antagonism of neuromuscular blockers muscle relaxation effects has also been included in the guidelines of the Chinese Anesthesia Society, greatly reducing the incidence of postoperative neuromuscular blockers residual side effects and improving patient safety undergoing anesthesia and operations.

Professor Yu is the editor in chief of the first "Anesthesiologists Clinical Quick Guide" (2014 version) by the CSA, the English version as the "Handbook of Clinical Anesthesia" which is the China's first internationally oriented distribution anesthesia guide. And the "Anesthesiology Terminology" of the Science and Technology Terminology Committee, as well as the  "Commonly Used Clinical Medical Terminology" (Second Edition) of the National Health Commission, that are the basic establishments of standardization in Chinese anesthesia discipline.

Professor Yu has established the national training system for grassroots anesthesia department directors as the president of Chinese Society of Anesthesiology, which has trained over 7000 grassroots anesthesia department directors, accounting for more than 50% of the total number of anesthesia department directors in China. This has rapidly improved the overall safety level of anesthesia in China, reducing the anesthesia mortality rate from one-5000th at the beginning of this century to the current level of one-50000th to 100000th, and below one-200000th in developed regions of China, reaching the level of developed countries. Guo Yanhong, then Deputy Director of the Medical Administration of the National Health Commission, specifically mentioned in an interview with CCTV in June 2018 that the quality and safety of anesthesia in China have continued to improve in recent years. The application of anesthesia technology has solved the problems of "dare not" and "cannot" surgery for people ranging from fetuses, newborns to centenarians. The mortality rate related to anesthesia is about 12 per million, significantly lower than the average level of 141 per million in developing countries and equivalent to around 10 per million in developed countries.

During his tenure as the president of Chinese Society of Anesthesiology, Professor Yu established the principle of "bilateral reciprocal exchange" with international academic organizations such as the United States, Germany, and the United Kingdom, leading a large number of Chinese anesthesiologists to the world stage and enhancing the international status of Chinese anesthesiology.

2、 Established the first international "Anesthesia Therapy Department" and pioneered a new field of anesthesiologists for treating diseases.

Professor Yu has established the world's first "Anesthesia Therapy Department" and innovated anesthesia treatment plans for refractory diseases, such as amyotrophic lateral sclersis (ALS), refractory psoriasis, severe sleep disorders, and refractory depression. The treatment effective rate for refractory psoriasis is over 70% (Dermatological Therapy 2021: e14866); The molecular mechanism of epidural injection of lidocaine for the treatment of psoriasis was first discovered internationally (The Journal of Investigative Dermatology, 2022:2173). The established field of anesthesia therapy has also become a leading international innovation field in the area of anesthesiology development.

3、 Developing an artificial intelligence assisted anesthesia robot system, Professor Yu aims to promote the future development of anesthesiology by artificial intelligence.

Professor Yu is pioneering the application of artificial intelligence in anesthesiology. He has developed multiple invention patents and innovative products, and established anesthesia drug delivery control methods and devices based on pulse neural network models, as well as brain activity state recognition methods and devices. Led the development of an automated artificial intelligence general anesthesia robot. Thus, it is possible to resolve the contradiction as the high-quality anesthesia services supplying and the current situation of the serious shortage of qualified anesthesiologists, and make up for regional differences in clinical anesthesia quality in China.

Professor Yu took the lead in proposing and continuously promoting anesthesia as the leading discipline in comfort medicine. Takes the lead in providing comfortable medical services such as "painless endoscopy" and painless labor. Promote the inclusion of childbirth analgesia in the national medical insurance payment program, ensure the safe and comfortable childbirth of mothers, and support the national population development strategy.

Professor Yu was the first to propose and establish a safety and comfort assurance discipline group based on the department of anesthesia. Taking the lead in proposing the establishment of new units such as anesthesia outpatient clinics, postoperative recovery rooms, anesthesia ICUs, and day surgeries. These suggestions and promotions have been adopted by the national health congress and have become the important contents of the National Health Office Medical Document [2019] No. 884. Ensuring the safe development of day surgery and enhanced recovery after surgery,.

4、 In the face of international difficulties in the treatment of critically ill SARS-CoV-2 infection patients, on behalf of the Chinese anesthesiologists, Professor Yu established the "suggestions from anesthesiologists for the prevention and treatment of COVID-19 pneumonia", developed the "quick guidelines for the treatment of critically ill SARS-CoV-2 infection patients by the anesthesiologists", and published the standard protocol as " anesthesia rescue therapies for severe and critically ill SARS-CoV-2 infection patients”, which have effectively improved the rescue and therapy success rate of SARS-CoV-2 infection patients.

When the "COVID-19" epidemic broke out in early 2020, Professor Yu first put forward and published the suggestions of anesthesiologists for the treatment of COVID-19 pneumonia (Shanghai Medical Journal 2020:102), established early tracheal intubation, sedation and analgesia, artificial hibernation, improvement of pulmonary microcirculation and other treatment strategies for critically ill patients, and have been written into the "Shanghai 2019 Expert Consensus on Comprehensive Treatment of Coronavirus Disease" in mid February 2020, The national health congress has adopted Professor Yu's suggestion to promote tracheal intubation treatment implemented by qualified anesthesiologists, greatly reducing the mortality rate of critically ill patients and effectively improving the success rate of resuscitation and treatment.

At the beginning of 2023, when the number of critically ill SARS-CoV-2 infection patients increased sharply, the "Quick Guide for Anesthesiology Treatment of Critically Ill SARS-CoV-2 infection patients " of the Chinese Anesthesiology Association and the group standards of the Chinese Medical Education Association were again written by Professor Yu, who led the anesthesiology departments of many large hospitals in China to carry out the rescue and treatment of critical illness, and achieved excellent results, as a success rate of more than 80%. The "Recommended Anesthesiology Protocol for the Treatment of Critically Ill SARS-CoV-2 Infection Patients " written by Professor Yu was published in the Chinese Journal of Emergency Medicine (2023:1347).

The theory of "Ideal State Anesthesia" and the practice standard of "Accuracy Anesthesia" created by Professor Yu were awarded the "Outstanding Contribution Award in Education Science and Technology" and the "First Prize in Science and Technology Innovation" by the China Medical Education Association in 2020. The 2017 "Medical Science and Technology Achievement Promotion Award" and the 2013 "Medical Science and Technology Second Prize" of the Shanghai Medical Association. Professor Yu has published over 500 papers, including 67 SCI indexed papers as the first author and corresponding author, with a total impact factor of 282 points and 1153 citations. In charge of three projects of the National Natural Science Foundation of China. Obtained dozens of invention patents, and edited in chief more than 15 professional reference books. Received the second "Shanghai Outstanding Specialist Physician Award" by the Shanghai Medical Association in 2016, the second "National Famous Doctor · Outstanding Achievement Award" in 2018, the nomination award for the second "Bethune style Good Doctor" in 2018, and the title of academician of the Russian Academy of Natural Sciences in 2021.

Professor Yu founded the "Flying Plan" and "Soaring Plan" of the Chinese Anesthesiology Society, cultivating a large number of young anesthesiologists from all over the country. He upholds the principles of "noble medical ethics, exquisite medical skills, honest medical performance, and elegance medical style", and throughout his life, he pursues the safety and health of patients, as well as the development and takeoff of Chinese Anesthesiology.
